Veterans’ Story Hour
Parent / guardian consent
Please complete this form and return it to the school before the visit. One form per child.
1. The visit
Veterans’ Story Hour brings Armed Forces veterans into the classroom to give a short, first-person account of service. It is not a parade and it is not a politics lesson.
- Length: one hour (one lesson).
- Speakers: two veterans.
- Stories: fifteen minutes each.
- Then: thirty minutes of questions and answers with the children.
School staff remain in the room throughout. The school stays in charge of the class. Speakers are asked to keep language and detail suitable for the year group. A child who does not take part will be given an alternative activity by the school.

Please tell the school in the usual way of any need the speakers should know about (for example hearing support). Do not write medical detail on this form unless the school asks you to.
4. Consent
Please tick one box in each section.

☐ I do not give permission. I understand the school will arrange an alternative activity.
The school or Veterans’ Story Hour may take photographs or short film of the session for the school newsletter, the campaign website, or reports to funders. Children will not be named in public material unless you agree below.

☐ I agree to my child’s first name being used with a photograph.
☐ I do not agree to photographs or film of my child being used outside the school.

By signing I confirm that I have parental responsibility for the child named above.
6. School use only
Notes for the school
This form is a template. The host school may put it on its own letterhead and keep the signed copies with its usual visit records.
- The school remains responsible for safeguarding during the visit. Staff stay in the room.
- Speaker names and a short outline of each account can be supplied in advance on request.
- Veterans’ Story Hour does not need a copy of this form unless the school is also sending photograph permissions for campaign use.
